Resource Navigator – Maternal Health Hub
Full time, non-exempt, 40 hours/week; Pay Rate: $17-$19/hour
Position Summary
The Resource Navigator supports women who are pregnant, postpartum, or parenting infants by helping them access services and resources that promote maternal, infant, and family well-being. Working within the Maternal Health Hub, this position provides person-centered navigation, outreach, education, referral coordination, and follow-up support to address barriers related to healthcare, insurance, transportation, housing, food access, benefits, childcare, behavioral health, and other social needs. The Resource Navigator builds trusting relationships with participants, collaborates with healthcare and community partners, and helps individuals move through complex systems with clarity and confidence. This position works out of the organization’s Rocktown Health office in downtown Harrisonburg and includes community-based work and home visits throughout surrounding areas.
Essential Functions and Responsibilities
- Provide direct navigation services to women who are pregnant, postpartum, or parenting infants and who may be experiencing barriers related to health, housing, transportation, benefits, childcare, behavioral health, substance use, or other social determinants of health.
- Conduct outreach and engagement in community settings, partner agencies, healthcare sites, and participant homes to connect individuals with Maternal Health Hub services.
- Complete screenings, intake activities, and participant needs assessments to identify strengths, priorities, barriers, and service needs.
- Support participants in developing individualized service plans focused on stability, health access, and family well-being.
- Provide information, education, and guided support related to prenatal care, postpartum care, pediatric care, family planning, insurance, WIC, Medicaid, SNAP, childcare, transportation, and other relevant services.
- Assist participants with completing applications, gathering documentation, scheduling appointments, and following through on referrals.
- Coordinate referrals and maintain communication with healthcare providers, case managers, social service agencies, and community partners, consistent with participant consent.
- Follow up with participants and referral partners to confirm service connection, identify unresolved barriers, and adjust support plans as needed.
- Provide advocacy and system navigation support to help participants understand options, ask questions, and make informed decisions about their care and services.
- Maintain an up-to-date working knowledge of local maternal health, pediatric, behavioral health, housing, benefits, legal, and family support resources.
- Collaborate with Maternal Health Hub staff and external partners to support coordinated, participant-centered care and reduce duplication of services.
- Document participant contacts, referrals, outcomes, and required data accurately and in a timely manner using agency systems and reporting tools.
- Participate in supervision, team meetings, case review, training, quality improvement, and program development activities.
- Maintain professional boundaries, confidentiality, and compliance with agency policies, participant consent requirements, and applicable regulations.
- Support community education, outreach events, and partnership-building efforts that increase awareness of Maternal Health Hub services and improve access to care.

- General computer proficiency, including email, internet navigation, spreadsheets, and electronic records systems
Required Qualifications
- High school diploma or equivalent required; associate or bachelor’s degree in human services, social work, public health, or a related field preferred
- Experience in resource navigation, community outreach, case management support, care coordination, social services, maternal health, or a related field
- Knowledge of or ability to quickly learn local healthcare, benefits, housing, transportation, and family support systems
- Valid driver’s license, reliable transportation, and acceptable driving record required
- Background check required
Preferred Qualifications
- Experience working with pregnant or postpartum women, infants, or families with young children
- Knowledge of maternal and infant health, public benefits enrollment, and community-based family support services
- Experience working in healthcare, nonprofit, community health, or collaborative care settings
- Spanish language skills
Work Environment and Physical Demands
- This position includes office-based and community-based work, including travel to participant homes, partner agencies, healthcare sites, and community events.
- The role may require extended periods of sitting, standing, walking, local travel, occasional lifting (50 Pounds) of program materials, and a flexible schedule that may include occasional evenings or weekends.

Licensed Professional Counselor
Part-Time, Approximately 20 hours/week, schedule negotiable; Pay range: $24-$26/hour
Position Summary
Strength In Peers, a peer-run Recovery Community Organization serving Harrisonburg City and Rockingham, Page, and Shenandoah Counties, is hiring a Licensed Professional Counselor. This part-time (approximately 50% FTE) exempt position has the possibility of expanding to full-time.
The Counselor provides clinical assessment, individual and group therapy, and care coordination for participants with substance use, mental health, and co-occurring challenges, and provides clinical supervision to our Registered Peer Recovery Specialists. Services are delivered in person and via telehealth at our New Market and Harrisonburg offices and at Page County Jail through our reentry program.
Essential Functions and Responsibilities
- Conduct clinical assessments and provide individual and group therapy using recovery-oriented, trauma-informed approaches
- Provide clinical supervision to Registered Peer Recovery Specialists, including sign-off on their progress notes and care plans
- Participate in care coordination with Peer Recovery Specialists and Case Management staff
- Support Medicaid billing compliance for therapy and peer recovery support services
- Deliver services at our New Market and Harrisonburg offices and at Page County Jail

- Genuine respect for peer support as a distinct discipline and comfort working within a peer-run organization
- Valid driver’s license and ability to travel among program sites
Preferred Qualifications
- Experience working with justice-involved, homeless, and other vulnerable populations
- Experience providing clinical supervision
- Experience with Peer Recovery Specialists or Recovery Community Organizations
- familiarity with Virginia Medicaid behavioral health billing
- Spanish language skills.
